The Banner ID  is a 9-digit number found on the first line of the mailing label (directly above your name) on the back cover of your Universitas Magazine or any other piece of mail from the Office of Alumni Relations.  Most Banner IDs begin with '000'.

Year Graduated is the year you received your first degree from Saint Louis University unless your subsequent degree(s) was(were) from one of the following schools: Medicine, Law, CADE, Social Service, Public Health or Dentistry.  In that case, please enter the year of your degree from those schools.

Date of Birth should be formatted without leading zeros on the month and date and using the 4-digit year.  For example,  January 1, 1950 would be entered 1/1/1950.
